Beauty, Hormones, and the O-Shot: A Modern Take on Perimenopause Care

Beyond Botox: Linda Mashkabov, a family nurse practitioner, takes a whole-body approach to aging.

Lyubov Mashkabova, who goes by Linda, is a family nurse practitioner who opened her medspa during the COVID-19 pandemic. It started with a beautiful space in the basement of her Fresh Meadows home, which is how the clinic got its name: The Beauty Cellar. “I began by offering Botox and microneedling to friends and family, and then word spread,” she says. Eventually, she outgrew her basement space and relocated to spacious new digs in New Hyde Park in spring 2025.

As her clientele grew, Mashkabov noticed a trend: The women who came to see her wanted more than wrinkle fixes—they wanted to feel like themselves again. “As women were sitting in my chair getting their Botox done, they would confide in me about what they were experiencing: brain fog, irritability, low libido, or what their husbands were experiencing—and their doctors told them everything was normal,” she says. “‘There had to be a better way,’ I thought.”

The Power of Plasma

Shortly after, Mashkabov was introduced to the work of Charles Runels, MD, the inventor of all platelet-rich plasma (PRP) treatments. “We draw your blood, spin it through a centrifuge to get your plasma, which has healing properties,” she says. PRP is used for collagen production (often paired with microneedling) and hair growth and restoration. It can be injected into joints to regenerate tissue. “And now it’s being used to improve sexual health,” says Mashkabov. Called the O-Shot for women, the plasma is injected in the clitoris or vaginal wall and it significantly improves vaginal dryness and urinary incontinence while increasing sensitivity. For men, it’s called the P-Shot, and it can help with mild erectile dysfunction. It’s your own plasma. “There are no synthetic hormones or chemicals—just regeneration from within.” It’s long-lasting, too. On average, the O-Shot lasts a year to 18 months and costs around $1,800; the P-shot lasts about 12 months and costs $1,500.

Targeted Hormonal Relief

Mashkabov decided to delve even deeper into hormonal aging and took a course on hormone replacement therapy (HRT). Now the clinic offers HRT pellets, small implants placed under the skin that deliver estradiol (a form of estrogen) or testosterone to the body. The dosage is based on your specific lab results, as well as your lifestyle and symptoms. Mashkabov says she recently got the pellets herself at age 42. “We’re not waiting for things to go downhill. We can be proactive and prevent certain things from happening later on,” she says.

A lot of women are surprised to hear they need to supplement testosterone. “It’s a hormone associated with males,” says Mashabov. “But in females, it helps with libido, sleep, irritability, brain fog—you can even have joint pain and weight gain from low testosterone,” she says.

Depending on your lab results, the pellets last about four to five months for women and three to four months for men and cost between $600 and $1,000.

As a family nurse practitioner, Mashkabov's work goes well beyond aesthetics. “I’m always educating my patients—even something like a vitamin D deficiency can eventually contribute to a lot of issues like dementia, Alzheimer’s, joint pain, and sleep.” The Beauty Cellar offers vitamin D injections and IV drips. “I love all things IV and injections because you get a hundred percent of what you are being injected with, whereas pills have to pass through your gut and you’re only getting about 30% of what you’re taking.”
